Health Department considers pilot Community Health Worker Program

The Graham County Health Department is beginning a strategic planning process aimed at implementing a pilot Community Health Worker program in the county. As part of the exploration and planning process the health department is asking community members to take part in a survey to understand what kinds of health issues people in Graham County face.

The survey is part of an assessment by the Graham County Health Department in collaboration with the Southeast Arizona Health Education Center (SEAHEC) and University of Arizona College of Public Health to learn about Graham County health priorities and how a Community Health Worker program might benefit its residents. The survey takes 5 minutes at most to take and is anonymous. The results will be shared with all county residents by the Graham County Health Department.
